Ayn Rand nailed it. The culprit is altruism. When the good is defined as sacrifice for others it does not matter how many times it results in failure. Once someone believes that sacrifice is the moral and the good it blinds them to the rest of reality. They are just doing what is good and if you disagree you are a Nazi and worse than Hitler!
