Many ages ago I taught at a Catholic school. The IT team prided themselves in making sure the computers were kid safe. One of my wild kids was in the library & typed in old ladies. And yes, old lady nudes appeared all over the screen. I laughed so hard when I found out.

I had 38 kids in my engineering technology class, each with a computer to do 3d design on.
What could go wrong?
actually not a lot as I had a unix box next to my desk acting as the dns server for my classroom, with only the approved websites used for the class addressed on it.
The number one question from the class, how come the internet is broken?
