That really puzzles me as well. I’m sure he could have had the highest quality and most effective treatments at any time that most likely would have prevented this amount of baldness. I guess it just wasn’t on his list of priorities, which is fine.

The press keeps calling her that. They also call her “Kate” which as far as I know is never used by her family or friends.
As a member of the Royal Family, she does not use a surname, so she probably has never had to choose between Middleton and Mountbatten-Windsor.
