Two possibilities exist: He was nuked by a moderator (a possibility I consider unlikely) or he self-nuked (asked to be removed).

2. Is CP OPH?

I've never seen any convincing evidence of that.

3. Why is CP not here now?

Consider it an impossibility. Established freepers cannot be nuked by a moderator. The software does not permit it.
