It's childs play actually.

When you have a post image starting with characters:
img src="https:

you simply insert
width="480"
in between the [img] and the [src]

which results in a string that starts:
img width="480" src="https:

[Bob Ireland]

True... the width=”480” can also follow the (url)” ...
