I wonder if off label Ivermectin would kill this thing?

According to Grok, yes. One of the strategies. A combination of physical removal, topical application, and injection.

it might:

Ivermectin is an effective systemic insecticide for New World Screwworm (NWS) (Cochliomyia hominivorax) infestations in livestock, providing residual protection against larval mortality and preventing infestations. While not approved for medicated feed in the US, it is used for individual animal treatment and as a part of import quarantine protocols. It can be administered orally or injected, with doses of 200 micrograms/kg achieving significant larval mortality and residual effects. However, ivermectin does not prevent other flies from laying eggs or reinfestation, making it part of a broader strategy that includes sterile fly releases and wound care.

maybe, it seems like a bit more on the macro infestation scale though, the larva drill or burrow deep into living tissue such as open wounds or soft flesh to escape and are actively eating the host alive.
