I don't know about the worst, but two of the best ever are Nirvana covering The Man Who Sold The World on MTV Unplugged and Metallica covering Whiskey in the Jar.

David Bowie did the first one and Thin Lizzy first did a modern version of the second one.

The Thin Lizzy version is great. Very bluesy. I alarm like Metallica’s version, which surprises me.
