It is supposed to be anonymous but only so many patients see a doctor and/or his staff so the dr. and/or their staff can figure out which patient’s commented in a time period. I waited a week and only responded because they had never sent a survey to me since I’d been a patient. IMO the hospital is more than likely getting paid for patients to take the Covid shot, just like they get paid more money for Covid hospital patients, so they were putting out feelers to find willing folks for the Covid shots.
I think you’re right about doctors and hospitals getting kickbacks for inoculations... probably have for a decade.
