Actually the sequence that’s repeated throughout the scene is;

1. Bell rings.

2. Shot of Tony looking at the door to see who came in.

3. The a POV shot showing what Tony sees.

So the ending was you see Meadow entering, with the bell, the quick shot of Tony looking up to see Meadow, then the blank POV shot.

It would have been more dramatic if Meadow suddenly had a look of shock on her face - as if she sees what is coming

Chase had made it clear in numerous interviews after the end of the show that Tony did not die.

In more recent interviews it’s clear he is sick and tired of being asked.

Did you notice that when Tony first walks into the diner, we see a shot of the inside the diner from Tonys POV.

BUT among the diner customers seated, is himself.
