That looks like a close-up of the shoulder of the female SS agent leaning in front of Trump as he raises his fist.
It does not look like the place for a button, so that is either an old jacket or maybe she narrowly escaped getting hit, herself.

If you go look at any other copy of that shot, you won’t see a hole. It is a poor photoshop job to boot.

That’s more the size of a .30 cal entry hole. 😆. Even a vest shot of that caliber would have knocked him off his feet. Its a button or something.

Could be a bullet hole (bullet bounced off kevlar vest), but more likely it’s a big drop of blood, staining the fabric.
