Actually it does provide for replacement of a president who becomes incapacitated while in office. The kicker for his detractors is that it requires the Vice President and a majority of the cabinet officers to invoke that provision.
Yes, they’ll first have to prove/establish that he’s impaired.

Section 4 allows for Congress to pass a law creating another body to make the determination that the President is incapacitated. If, in their wildest dreams, the Democrats had control of both houses of Congress in 2019, it’s virtually certain they would not have a veto-proof majority.
