What does it take to appoint a special independent federal investigator/prosecutor? I’ve wondered that for a long time. Who has to authorize that?

Who has to authorize that? If things have not changed since Nixon then a Special Prosecutor serves at the digression of the President. Nixon fired his Special Prosecutor investigating Nixon (iirc). Did not hold (the firing) for people went nuts but was tried (iirc).
