I think we've seen from auto-penning to Carney saying that Hillary's signature on a document doesn't mean she saw the document, that this administration has no regard for "only" or "authorize" or any other chain-of-command requirements.
I still wonder if Obama was absent and others (Jarrett?) gave "orders" that were followed in Obama's name?

Pretty close. Unless the CIC orders a cross border excursion, the default standing order is no-go. A no-go isn't a matter of issuance, it's the standing order until the GO signal is given, and the golfer-in-chief couldn't be bothered with it. He had some campaigning in Las Vegas on his
agenda.
