if he was known to authorities, on a terrorist list, and “radicalized”, why was he walking around the country free and able to commit this massacre?

To me it means he got religion, and decided to do what his God (Allah) wants done.
These Kafirs(Infidels) are not going to convert to Islam, and they are not going to pay protection money (Jizya) so they should be killed.
That is what Allah wants , that is the example set by Allah's messenger Muhammad, and that is what fundamental Muslims have done for the last 1400 years. - Tom
