This is hardly the first time that something like this has happened.
News agencies have long kept updated obituaries for high-profile people on file they've mistakenly been released in the past (in 2003 CNN's website accidentally released obituaries for Presidents Reagan and Ford, Vice President Cheney, Pope John Paul II, Nelson Mandela and Fidel Castro). The problem with things like Twitter is that it can go viral within seconds.

This is standard UK media practice, for the Royals and elderly major politicians. They did it yearly for the Queen Mum for over 20 yrs.
This one just went wrong.
