I want to know why the IRS didn’t treat that as a security issue and possible hack? How did the players know it wasn’t a hostile act from an outside source?

Exactly right. When asked specifically why data was not recovered from the incremental (tape) backup, Koskinen opined that was not done probably because it was too much work.
I’ve worked with several federal agencies. These people are full of crap. When they found that her disk had “crashed” they had near time copies on media. They could have restored the data easily. Or, if there was a problem they could have simply copied the tape and set it aside.
