I have firsthand knowledge of a sperm donor's name being removed and the adopting father's name being added to a birth certificate when a child was adopted at age 5.

The resultant birth certificate (and any future passport applications, FBI checks etc.) consider the adopting father to be the child's "real" father.

No "original" birth certificate (listing the sperm donor) is available from any governmental agency.
