Normally, I'd say it was probably something as simple as having the wrong crypto loaded into the radio gear or some other PEBKAC (problem exists between keyboard and chair), but that part about the red flare really gets my attention, that is very unusual.
Lost comms? Navy called rescue? All OK now, no problems? There is going to be someone on the carpet for this.
