A similar thing happened to me once, but in my case the lasagna was kind enough to warn me of it's intentions prior to exploding.Good thing it did. Probably saved you a lot of mental anguish.

Always read the directions: Like on ethnic chinese food it says to let it set on your plate 2 minutes. It it starts to crawl away it’s not done.

Obviously, it had been microwaved. Restaurants should prick or stir any dish that comes out of their microwaves before being sent out.

I nuke eggs for breakfast in a ‘tainer made for the purpose. While in the microwave it will go KABOOM! at least once.
Anyway, wish this was a case of Russian reversal as in,
“In America you get lasagna. In Russia, lasagna gets you.”
Oh, well.

Probably from a leftover pan of lasagna the night before. Tacky.
We had phenomenally great homemade lasagna. Like chili, it tastes as good or better when reheated the next day.

Happened to me at work years ago. Trying to eat cheaply on a budget so I took in a couple of hard boiled eggs for lunch one day. The first bite I took was fine, because I hadn’t gotten to the yolk. The second bite was the one that did it. That egg disappeared in a cloud of white and yellow, burning my lip in the process. I stumbled out of the break room, with bits of egg in my hair and a fat lip, and mumbled something like, “My...egg...blew up”, while the rest of the office collapsed into laughter.
That was a short lunch break.

I cracked an egg into a bowl and microwaved it. It came out the prettiest sunny side up egg I ever saw. Until I put my fork in it. I had to clean egg off of me, the ceiling, and the walls.

The sauce can't be all that good if there's enough free water to allow nucleation.
I thought the resting period (of nuked food) was to even out any hot spots in the food for a better end product.

That's another reason the "Eggtastic" instructions on YT say to at least pierce the yolk(s), if not scrambling.
I had to clean the MW the other morning. I nuked a couple of eggs, pierced the yolks, but forgot to cover the tray with a paper towel (I use a greased ramen noodle MW tray for eggs when lazy).
Some of the egg whites exploded into cooked granules, for the most part.
