What happens to the soul of a baby (or anyone) born into a Christian household, but neither baptized nor confirmed / born again before dying? Are last rites / the Sacrament of Extreme Unction sufficient to enter Heaven?

Yes, and often, when a baby is dying, the baby will more often then not, be baptized in an emergency manner.

Understand, thanks. What of unbaptized individuals who expire before a priest, minister, or pastor can be present — a stillborn baby, a child who has drowned, a teenager killed in an auto accident, a young adult who suffers sudden cardiac arrest? Is baptism after the fact acceptable? Has the soul already departed the body, and if so, where is it? Are last rites / Extreme Unction effective?
