When I see people like this guy who have disfigured themselves with tattoos, studs on their lips and face, I take them to be someone who has no self worth, hates themselves and wants the whole world to know it.
A modest amount of tattoos and one lip stud is a “look at me see how cool I am” kind of thing but someone who keeps getting more and more is one sick person.

Or it could be that he was just born that way, and nothing “bad” happened to him.
Some people are just born wired up “wrong” for lack of a better phrase or term. I have known several people like that.
