You nailed it. I’m overweight by 30 lbs but daily exercise (one hour of bike riding) has cleaned up my blood chemistry.
Obesity does NOT mean unhealthy (at least for me).
I’m still working on the overeating, which seems to be a lifelong behavioral struggle.
Take a flight to Indi. Almost every drug imaginable is on the back of a bike pharmacy cruising through the poor parts of town. Not saying I would recommend a super dangerous drug like Ivermectin twelve 12mg tablets for $2.,,, Or saving money on any other drug because our poor pharma companies faired so hard during the pandemic.... But with those prices you can take a nice trip to Pondy or Goa and see a doc there and get drugs on the cheap.

I suspect Eliquis from the prices you mentioned, I found a similar problem a few years back.
I get my first two prescriptions filled in the US and they cost me $135 for a 90 days supply with medicare part B paying for most of the cost. Then after two prescriptions the cost goes up to about $500 for a 90 day supply because my deductible is reached.
So then I get my next two prescriptions from India (generic Apixaban) at a cost of $141 fo a 90 day supply (including shipping). Been doing this for two years now and so far the service and product has been good. Takes about 3-4 weeks to get. They will need a copy of the prescription. The Apixaban they ship is made by a company called Natco, a major drug manufacturer in India.
My cardiologist is OK with me doing this, he writes me two prescription, one for the local pharmacy, one for affordable RX.

Usually about 15 to 25% of the US prices. You can choose what country they are made. We usually got them from India. Give them a call or visit their website.

Perhaps contact the manufacturer and see if they have a, “manufacturer’s coupon” that will handily discount the medocation. Most manufacturers offer one, as for instance there is a special program for Ozempic.

I examined the plastic bottle carefully, and there is no mention of where made. Only thing it says is Centerwell Pharmacy from Ohio. My BP is behaving so may be the meds is working.
India population is approaching five times bigger than United States. Does not indicate any mass killings by India made drugs. Pharmaceuticals have been a big industry in India since I was a child growing up there. That was in 1940’s and 1950’s. Arrived in US in 1960 to work on master’s in mechanical engineering. My employers talked me into staying and I obeyed. Still here at age 83 and in amazing shape from regular exercise. Here is a recent video from 2023.

Glad to hear such a low dose is working for you. Or do you need it it all? A friend was trying to find a replacement for his Lisinopril that isn’t made in China and none could be found. Here’s who makes Lisinopril and the FDA alert for this company that got my friend’s attention. His doctor knew nothing about it, etc etc
