#Buprenorphine drugs, like #Suboxone and #Subutex, have been heavily marketed as the wonder drug for #opiate & #heroin #AddictionTreatment. While buprenorphine is one of 3 drug classes used in the SAMHSA evidence-based best practice, Medication Assisted Treatment, buprenorphine plateaus in effectiveness at an intermediate level of dependency, failing to manage withdrawals and cravings for intermediate to heavy dependencies.

Additionally, the vast majority of buprenorphine treatment has been "in the privacy of a doctor's office without the need for daily visits," as it was heavily marketed. This often means a doctor writes a 30 day prescription warning the patient:

1) Don't take other drugs with this. 2) Get counseling.

However, it is easy to understand why this seldom occurs. So, it is a drug that fails to adequately manage withdrawals and cravings for intermediate to heavy dependencies, and most often fails to provide:

1) Treatment for the underlying addiction. 2) Clinical accountability.

It is easy to understand why treatment outcomes have proven dismal for intermediate to heavy dependencies.

For such persons, #methadone has been found to be most effective. Hear the stories of persons with multiple failed treatment attempts who made that transition in the video below.

But not all #MethadoneClinics are the same. You need to look for the following characteristics in a program:

1) Group and individual psychotherapy at least 3 times per week in the initial phase of treatment utilizing research evidence-based best practices skillfully.

2) Staff who are warm, welcoming, and respectful. This should not need be said but it does.

3) Skillful Co-Occurring capable clinicians. Opiates tend to be the drug of choice for persons with mental illnesses, especially those with trauma histories. Traditional substance abuse confrontational approaches are harmful. Different counseling approaches and techniques are advisable. Likewise, mental illness symptoms can be mis-read.

4) Subsequent physician visits to effectively titrate dosing to optimal levels.

5) Frequent and random drug testing utilizing more expensive test instruments to detect temperature, adulterants, and at least 12 drug panels. These should be backed up by lab confirmations.

6) Care coordination with primary care and mental health providers.

My assertions are based upon interviewing heroin addicts as part of my job....but it wouldn’t be the 1st time I was wrong.

But if I were allowed to share my video recorded interviews you would see heroin addicts cursing when asked about methodone clinics in my area.

Our politicians in Seattle area are setting uo “safe injection sites” where addicts can shoot up without being arrested.

The addicts I interview, and I agree with them, say this will increase addiction and be just like the methadone clinics....places for dealers to congregate (within a block or 2) and do deals.

I think public money for methadone clinics would be better spent on subsidizing rehab beds.

Every person I have tried to assist gtting into rehab was on state assisted health insurance (Apple insurance here in WA.)

I call the clinics and they say “Yes we have 6 (or more) open beds right now” He/she has Apple insurance. “Oh...we only have 2 dedicated beds for Apple insurance and tjey are full...booked up out 14 months.”

If you have exoerience with heroin addicts you know the moment they are ready to go into treatment you have to swoop in. If there is a significant delay the pain and the monster starts telling them they cannot do it and they fall back down in the well.

When they next are ready to hit treatment could be in 3 days or 3 years.

I actually interviewed a local elected official and asked them why we are spending money on safe injection sights. The answer was of course no answer.

I think my local politicians would rather be “on the cutting edge” and say “we are the 1st in the nation” to domsomething rather than actually save people.

Not any more. The drug industry has solved that problem with Narcan.
Drug addicts can get even more deadly highs and be brought back, over and over.

For most opiate ODs we have removed death as a consequence.

Druggies couldn't be happier. We can now go back to the important work of enabling the addict population to grow even larger. And we are succeeding, you can’t believe how well we are succeeding.

All that is is an entire industry of people making a living off of not curing a problem that only the addict and God can cure.

All that one needs to cure one’s addiction is the will to conquer the demon, the desire to be free and the Strength of God to overcome.

Yes, there will be suffering.

But as an addict, I am the one who made my prison, walked into it and locked the door behind me. I am the only one who can unlock the door, walk out and never return.

My addiction/ addictions/ addictive behavior is between me and myself and God, and no one else.

I am sober. My demon is in my prison where I put it. I locked the bastard in there. I hold the key right here. It’s up to me to never let it out. I’m the only one who can do this job.

That is all.

alcoholics and drug abusers can safely be lead thru detox with a methadone taper or a Librium taper over a few days, with other meds for anxiety, sleep, achiness, etc...it doesn't have to be a nightmare....

if they follow that up with a long inpatient rehab, they have a chance....

the biggest thing is they must not rejoin their social group...

Sounds like you know from personal experience. I happen to agree with you. I wish the gun toting feds would leave our medical choices to the doctor and patient. There is nothing worse for a good clinician than to place his license in the hands of a fascistic anti-freedom all-powerful agent from the government.

You’re right. Pain is SO ###ING subjective!

My head hurt so badly i wanted to die. But there was no proof.

But the oxy’s didn’t help at all. even at 240 a day.

After the suboxone incident, i came off everything.

never got a high once. Maybe felt a little more light hearted for five minutes after a dose. i guess that’s a high

Yes they can!

5-8 days detox and straight into rehab for 30 to 90 days depending on the individual. Kick the physical addiction them work on the mental. If the individual goes in on their own your looking at 75 to 80 percent success rate. We use a 12 step based rehab and will put them in a Sober Living facility if needed, also AA based.

If he was an opiate addict, he wouldn’t die, detox is like a bad flu

Meth addicts have it a bit harder.

If he was also an alcohol addict he had about a 35 to 40% chance of dying without treatment.
