...after a spare lithium ion battery for her e-cigarette exploded in her pocket.

Did she have a cap on the connection end of the spare battery?

When I remove the cartridge from the battery on mine, the battery's terminals are exposed. They could possibly have been shorted out by some sort of metallic object in her pocket.

My first thought was yours: keys or some other metal object in her pocket.
